The DVA Community Nursing Program provides essential in-home nursing care to support veterans and war widows/widowers with their clinical and personal care needs. This program enables individuals to maintain their independence, improve their wellbeing, and receive professional medical support at home.

Are You Eligible?

If you are a veteran or war widow/widower holding a DVA Gold or White Card, you may be eligible for government-funded community nursing services.

An assessment is required to determine your clinical and/or personal care needs.

To access services, you will need a referral from one of the following:

  • General Practitioner (GP)

  • Nurse Practitioner (Community Nursing)

  • Treating Hospital Doctor

  • Hospital Discharge Planner

Getting Started

Once a referral is received, we will:

  • Arrange a free in-home consultation at a convenient time

  • Guide you through the assessment process

  • Connect you with our experienced Registered Nurses for personalised care
